The State of California has established a Community Care Expansion (CCE) program (reference Assembly Bill 172 (Chapter 696, Statutes of 2021)) to promote the sustainability of residential adult and senior care facilities and to address historic gaps in the long-term care continuum.  Funding is available through the State’s CCE Preservation Fund to immediately preserve and avoid the closure of licensed residential adult and senior care facilities serving applicants or recipients of SSI/SSP or CAPI, including those who are experiencing or are at risk of homelessness.  The CCE Preservation Funds are divided into two components.  Operating Subsidy Payments (OSP) are available to fund operating subsidies to existing licensed residential adult and senior care facilities serving qualified residents.  Capital Projects (CP) funds are available to preserve facilities in need of critical repairs or required upgrades.

Placer County has obtained CCE funding for distribution to qualifying facilities within Placer County.  Facilities who wish to apply for these funds must meet the requirements specified in Section 4.0 of Attachment A.  Qualified applicants must complete and return the Grant Application form provided in Attachment B, as well as all associated forms provided in Attachments C, D, and E.
